Be sure to fill out your Yellow, Red and Blue Forms and ensure they are sent in with your game reports within 24 hours.
To ensure disciplinary action is taken, the NSA MUST receive these forms immediately !
Keep in mind, if a player receives 3 yellow cards in a single playing season, they may be subject to disciplinary action.
Review your Laws for Red and Yellow cards often.
Not giving out cards during a game when they are warranted only sends the wrong message to the teams that they can get away with offences.
Hopefully you will never have to fill out a referee assault form.
First Rule of Assaults: Contact the Niagara Branch (SRN) for assistance. This applies to all members, from Class 4 to Class 1 and above. Don't send in your report until someone else has reviewed it with you.
Use the Game Official Assault Form to report any incident involving physical contact or threats to the game officials during or after a game.
The 2006 renewal form is on the site...like the other forms, it will show up on your computer as a .PDF form for you to print out. If you do not have a .PDF reader, there is a link at the top of the page where you can get one for free.
This form must be submitted to the appropriate authority within 48 hours of the game or earlier if stipulated by the rules of the competition. This form is to be used to report a special incident that is not covered by a Referee Report Caution Form, a Referee Report Dismissal Form, or a Referee Assault Report Form (e.g. abandoned game, reporting misconduct by a Coach, outside interference by spectators). In any case involving physical contact with a game official, the Referee Assault Report Form should be used.
Referee Report - Dismissal Form
This form must be used by a referee for each dismissal issued. This form must be submitted to the appropriate authority within 48 hours of the game or earlier if stipulated by the rules of the competition. In any case involving physical contact with a game official, the Referee Assault Report Form must be used.
Referee Report - Caution Form
If a player is sent off for a second cautionable offence, the dismissal form must include a short description of the first cautionable offence.

These forms must be used by referees when reporting an assault on the referee or either of the linesmen. If a player has cautioned, a caution form must also be completed. If a player has been dismissed, a dismissal form must also be completed. This form must be submitted to organizations listed below within 48 hours of the game.
